2023-24 Student Registration

A huge welcome to all incoming families!

If your child has received an offer to PS 15 please complete the registration process now to guarantee your seat. The process involves three registration forms to fill out, and personal documents (proof of age, residency and immunization).

1. You can download and fill out the forms below or contact the Main Office to complete them at you registration appointment.

2. You can request assistance from one of our office staff. To request an appointment for assistance, email or leave a message with Melissa at 917-669-0508 or Barbara at 718-330-9280, and one of our office staff will contact you to set up an appointment (office personnel are checking the voicemail messages during the school day). For incoming 3K, PreK or Kindergarten, if you applied and received an acceptance letter or waitlist, we will reach out to set up an appointment for you.

Pre-Registration requirements

Home Language Identification Survey, available for download in multiple languages, or within the PS 15 Pre-Registration Questionnaire
Documents to submit:
a. Proof of Age (birth certificate, passport, baptism records)
b. Immunization records (if available)
c. Proof of residency (please refer to this preregistration checklist for eligible documents)

To submit these documents, you can scan or take a picture and upload them within the PS 15 Pre-Registration Questionnaire (if you have a gmail/google account), or email them directly to 15k015@schools.nyc.gov.
