If your child has received an offer to PS 15 please complete the registration process now to guarantee your seat. The process involves three registration forms to fill out, and personal documents (proof of age, residency and immunization) to send to the school. Please choose the way which is easiest for you to submit them from one of the options below:
1. You can download and fill out the forms (see list below), and save pictures of the other documents and upload them all into PS 15 Pre-Registration Questionnaire. This option may require you to have a google (gmail) account.
2. You can answer the questions from the forms online through PS 15 Pre-Registration Questionnaire (please allow ample time), and independently email copies/images of available documents to firstname.lastname@example.org (if emailing, always include the name and grade of your child in the subject line).
3. You can request assistance from one of our office staff. To request an appointment for assistance, email or leave a message on the school phone (718 330 9280), and one of our office staff will contact you to set up an appointment (office personnel are checking the voicemail messages during the school day).
If you need the forms in a different language please contact our parent coordinator Ms. Melissa Johnson by email or at 917-669-0508 We recognize these are difficult times and you may not be able to provide all required information now. Please do your best and office staff will follow up.
